Terms
- Each plan includes one comprehensive maintenance visit per year.
- Plans automatically renew unless written notice is given at least 30 days prior to the end of the term.
- 10% labor discount applies to all repair work performed by A.W. Crooks during the active term.
- Standard trip / diagnostic fee is waived for plan members on covered service calls.
- Plan covers preventive maintenance only. Parts, repairs, replacements, and excavation are extra.
- Customer must provide safe access to all plumbing systems and secure pets on the scheduled date.
- Exclusions: slab leaks, main sewer collapse, freeze damage, neglect, and pre-existing conditions.
- Either party may cancel with 30 days written notice. Early cancellation may forfeit remaining discounts.
- Provider’s liability is limited to the annual fee paid. This agreement is governed by Michigan law.

- Either party may terminate with 60 days written notice.
